Yeah, I know--screen shots, your moniter size and all that stuff--but I am trying to make it readable too----I will attempt to incorporate your question in the comments that I make as we go along. (Eventually, I will add a spreadsheet that you can download.)
A general cost center would look like this
Prepress costs would show up here. Return to the opening screen.
Folder costs are determined like this. The same is true for cutters. Return to the opening screen.

Administrative costs must be calculated. Use this approach and then spread them over the various cost centers based on a percentage. Return to the opening screen.
